Disable ShootQ Contact Form in Facebook

It's simple to hide the ShootQ contact form in Facebook, just follow these instructions.

Go to your business' Facebook page and click Settings.

Click Apps and click the "X" to the right of the ShootQ contact form app to remove it. 

In the window that appears, click Remove. 

Now the ShootQ Contact Form will no longer be accessible on your page. To add the ShootQ Contact Form again, go back to your ShootQ settings and click Facebook under "Integrations" at the bottom of the page.
